    RESIDENT FOUND WITH THE RESDIENT'S HEAD BETWEEN THE BED AND THE RAIL. THE RESIDENT'S LOWER BODY WAS OFF THE BED. THE RESIDENT WAS NOT BREATHING AND NO VITAL SIGNS.
