    RESIDENT WAS FOUND WITH HEAD AND NECK CAUGHT BETWEEN RAILS AND MATTRESS. RESIDENT DID ROUTINELY USE THE SIDERAIL TO ASSIST IN REPOSITIONING. AUTOPSY REPORTED ASPHYXIATION CAUSE OF DEATH.
