    WHILE PERFORMING A TRANSURETHRAL RESECTION OF A BLADDER NECK STRICTURE, THE COLD KNIFE BROKE OFF 1/2 WAY DOWN THE SHAFT AFTER THE PHYSICIAN BEGAN TO CUT TISSUE. THE BROKEN END WAS RETRIEVED FROM THE PT'S BLADDER WITHOUT INJURY.
