    A PT, WHO HAD FALLEN, WAS TRANSPORTED TO THE DEM ON A STRETCHER. AFTER ARRIVING IN THE DEM A WHEEL ASSEMBLY OF STRETCHER BROKE APART CAUSING THE PT TO FALL TO THE GROUND.
