    THIS INVOLVES A STAFF MEMBER, NOT A PT. AS SHE WAS REMOVING THE WAND FROM THE BLEACH BOTTLE, THE LINE POPPED OFF THE DIALYSIS MACHINE, CAUSING BLEACH TO SPRAY UNDER HER FACE SHIELD & INTO BOTH EYES.
