    ON 2 RECENT OCCASIONS THE JAR HAS BLOWN THE LID AND CLAMP OFF AS THE MEDICAL TECHNOLOGIST WAS TIGHTENING THE CLAMP ON THE JAR. (PROPER PROCEDURE). PERSONNEL WERE HIT WITH LID AND/OR CLAMP. BOTH CLAMPS BROKE.
