    CONSUMER REMOVED A TAMPON AND THE TAMPON CAME APART INSIDE HER. SHE REMOVED MULTIPLE TAMPON PIECES THAT HAD REMAINED INSIDE. THIS IS A NON-US PRODUCT MALFUNCTION. THIS EVENT OCCURRED IN (B)(4).
