THE CUSTOMER REPORTED THAT THE O-RINGS WERE CROCKED AND INSULIN WAS LEAKING PAST THE O-RINGS. THE CUSTOMER STATED THAT THE RESERVOIR'S SEPTUM WAS SMOOTH AND FLAT. NO FURTHER INFO WAS PROVIDED.
