THE CUSTOMER STATED THAT THE RESERVOIR LEAKED INSULIN PASSED THE O-RINGS. THE CUSTOMER STATED THAT ONE OF THE O-RINGS LOOKED DISPLACED. NOTHING FURTHER WAS REPORTED.
