The device is intended to be used by hospital staff to estimate the risk of potentially avoidable 30-day readmission for hospitalized patients who are about to be discharged, especially cardiac patients. The device combines hemoglobin at discharge, discharge from oncology service, sodium level at discharge, procedure during index admission, index type of admission, number of admissions in the past 12 months and length of stay to calculate the estimated risk of preventable 30-day readmission
